Using the following chart, record and observe patterns using the suggested monitors. Add any symptoms you're experiencing in the blank spaces and score them also. Use the journal to record periods, dietary intake, and any other potential triggers. It is important to log your symptoms every day if possible, at the same time of day. Score the symptoms by severity from 0 to 3, 0 being none and 3 being most severe. If you can, begin your journal one month before you go to your checkup, and continue keeping it as your therapy begins. Use the back of the journal to keep notes on your appointments, medications or change of medications, doses, side effects, and any new health problems.
